The following documents MUST be uploaded at the time of pre enrollment.

1. PROOF OF BIRTH (Birth Certificate, passport, Hospital Record, Baptism Certificate)

2. IMMUNIZATIONS RECORDS (Minimum Immunization Requirements)

3. PROOF OF TB WITH DATE GIVEN, DATE READ, AND WRITTEN RESULTS (Must be within 15 months) *ONLY if your child has recently been out of the country for 1 year or longer.  

4. PROOF OF CURRENT GRADE LEVEL (If grade is not age appropriate. Proof would include current report card or other documents from the prior school indicating grade level.)

5. INDIVIDUALIZED EDUCATION PLAN (IEP), IF APPLICABLE

6. CURRENT PHOTO IDENTIFICATION OF PARENT/GUARDIAN

7. VERIFICATION OF RESIDENCE (current utility bill: water, gas, electric, or disposal services ONLY)

8. Oral Health Assessment (select "add" under documents section)

9. Report of Health Examination for School Entry (select "add" under documents section)

• If living in the residence of another person, that person must accompany the parent at the time of registration and present their utility bill and current photo ID. Both parties must complete the affidavit stipulating the registering parent lives in the residence of another person. The parent must present their photo ID and then may register.

Medication Drop Off Procedure:

Medications can be dropped off in the health office at any time and must have a NEW Authorization For Medication Form completed by a physician for each medication needed at school. Please click on the link below for the required form that is needed.
